Over the coming months it is intended to provide a link to a pdf or Microsoft Word version of the many forms we use in the district. These can then be downloaded by leaders and issued to their members or if a parent has lost a form they can be given given this web page so that they can download another copy for themselves. If any leader wants a copy in .doc format so they can enter their own unique details please contact the DC who will e-mail a copy to you. New Members Registration form - This form can be used to collect the information needed by their section leader. It includes name, address, telephone numbers, allergies and other medical information. New member information - This gives a new member a lot of useful information about Scouting and contact phone numbers for the leaders in the group. Parents Permission to take part in a Scouting Activity - This includes attendance at a camp, sleepover or just a day trip etc. The form contains details of the activity and has provision for the parent or guardian to give their permission for the child to attend the activity. It also asks them to confirm any changes to the medical information a group holds about the child. Nights Away Notification - This form is used by a leader to notify the DC about a forthcoming Nights Away Experience. The form should be sent to the DC at least one week before the activity takes place.
